When a health and safety incident occurs in a workplace, a thorough investigation is required to determine the causal factors, establish corrective actions and preventive measures to minimise or eliminate any current or future risk.
It is important that health and safety investigations are never conducted simply to apportion blame or liability for what occurred.
A health and safety incident investigation must be methodical and structured, with an unbiased approach to the information gathering, collation and analysis. Construct Health will thoroughly investigate any incidents, accidents, and reported cases of occupational illness, and will identify corrective measures for any deficiencies in the health and safety management systems, failed defences, organisational factors, and intentional or unintentional human errors. We will also establish your level of legal compliance, and improve your overall Safety Management Systems as a result of the findings.
An ICAM investigation was commissioned by Auckland Regional Dental Services in response to foaming liquid being identified coming from dental air supply. This entailed working closely with several district health boards, their engineering teams, and working with ARDS employees to ascertain the root cause through a no blame process. This was successfully completed and issued to the Ministry of Health.
